Following your brief video Meet & Greet, we’ll schedule your child’s in-home visit.
From well exams and vaccines to sick care and ongoing support, your provider comes directly to your home to assess, diagnose, and provide personalized primary care — all in the comfort of your own space.
All paperwork must be completed prior to the visit, otherwise the visit may have to be rescheduled.
What Happens Next?

• We review findings and create a personalized care plan
• We schedule upcoming well visits and vaccines as needed
• You receive direct communication access for questions or concerns
• We coordinate referrals, labs, or specialists if necessary
• Sick visits are handled promptly via telehealth or in-home care

Protecting your teen's safety and privacy
What does that mean?
Up until 12 years old: In the Portal, parents/legal guardians create an account and schedule directly there or send us an appointment request.*
After 12 years old: In the Portal, a minor's email and contact information is needed. From 12-15 years old, parents/legal guardians will have limited access to the Portal until the minor's 15th birthday, per State law.*
